Win/Loss Statements will be available on the Four Queens website, www.fourqueens.com, and in person at the Royal Players ClubTM.

Win/Loss Statements will only be processed for the current year.

All requests, whether received by mail or in person, for 2006 will be processed starting January 2007.

All requests that are mailed must be the original notarized form.

We will not accept email or faxed requests.

Valid photo identification will be required for all requests brought to the Royal Players ClubTM, that have not been notarized.
